ATW should not reject your claim because you have used the old form.
If you send in a claim form using the old form (two pages) you may have it returned because you have used the old form. ATW have said they should not return it, but should pay it, and then send you the new claim form for you to use next time. Let DeafATW know if ATW return your claim form without paying it.
